What is Connections?
Colonial Behavioral Health (CBH) operates two Medicaid Waiver group homes for adults with developmental disabilities known as the “Connections” program. The Connections program assists residents with living independently in the community to the greatest extent possible. Based on individual need, instructional activities and staff support are provided to help individuals improve the quality of their daily lives and promote self-determination, wellness, and community integration.
How does Connections work?
CBH’s Connections group homes are located in neighborhoods in the Greater Williamsburg area that are close to community resources such as restaurants, hospitals, shopping areas, and more, which affords the residents community integration on an ongoing basis. Four residents live in each group home and each individual has his/her own private bedroom. Program staff is on site at all times when the residents are home to assist with daily activities and personal care needs. Skills training services and community supports are provided based on a person-centered plan that is developed with the individual, family, and provider team. Wellness, support, and safety preferences are incorporated into the planning process. Admission to this program requires a Community Living Medicaid Waiver plan.

Adults with developmental disabilities who have a Medicaid Waiver, specifically a Community Living Waiver.
What services are provided in the program?
Residential Services may include the following based on individual need:
- Functional skills training to promote independent living
- Supervision to ensure the individual's health and safety
- Assistance with personal care and use of community resources
- Monitoring health and physical conditions
- Medication management
- Assistance with medical appointments as needed
