A community-based residential program for adults with developmental disabilities.

Residential services are tailored to the needs of each individual based on an individualized support plan that the resident is involved with creating, with an emphasis on gaining independence.

Opportunities for maintaining family relationships, peer interaction, community involvement and social interactions are also available. Pursuit of hobbies and interests is encouraged.

We are committed to the rights and responsibilities of individuals, honoring individual choice, and encouraging community participation.

Community Living Waiver

A Community Living Waiver is required. The Waiver pays for the service provided by residential staff.

How Do I Access Connections Services?

Your Case Manager/Support Coordinator will discuss your residential options and send a referral to Connections with your agreement.

We also accept referrals from Case Managers/Support Coordinators outside CBH's service area.

What Training is Required of Staff?

Residential staff are trained in a variety of topics, including Human Rights, HCBS Rights, various Health and Safety topics, and Person-Centered Planning.
